POSTAGE AND HANDLING: Please add $4.00 for the first book and $1.00 for
each additional book.
TEXANS: Please add 8.25% Sales Tax to your order.

Your shipping address including zip+4
List one book per line, each book you would like to order with the following
information:
Order # from the catalog
Name of book
Quantity desired
Price
You may ENCLOSE your order information when you send your check, or if you
would like me to prepare your order for shipment so it is ready to go when your
payment arrives, email the above information to me or you may print it out and
fax it to our fax number, 936-552-8999.
